Join the AstraZeneca HealthCare Foundation (AZHCF) for a moderated roundtable discussion via livestream to hear grant awardees share their viewpoint on developing and implementing community-based programs to improve cardiovascular health. Ten U.S.-based nonprofit organizations that received funding through the AZHCF's Connections for Cardiovascular HealthSM program will share lessons learned from their programs, which include both rural and urban programs from New York to California. Viewers will also hear the success stories of individuals who have participated in some of these programs.
These organizations are working in innovative ways to address the urgent, unmet needs of their communities including such approaches as leveraging access to care for uninsured and underserved populations, bringing programs directly to participants where they live, work and play, using health coaches to provide one-on-one support and education, and implementing culturally-sensitive program interventions.
